The Texas A&M University-Commerce (TAMUC) Occupational Health and Safety Program is dedicated to providing a program to protect the health and safety of its employees, staff and students. The purpose and goal of the program is to identify, evaluate, manage and reduce potential health risks associated with the TAMUC environment and to comply with federal and state regulations.

TAMUC Occupational Health and Safety Program enrollment is required for all individuals who work directly or indirectly with pathogens, animals, animal tissues or wastes, chemicals, physical hazards and those that are identified as Bloodborne Pathogens (http://www.tamuc.edu/facultyStaffServices/riskManagementSafety/healthAndSafety/bloodbornePathogens.aspx)

Examples of individuals who are required to participate include:

• Individuals listed on the Institutional Biosafety Committee research permit working with agents at BSL2
• Individuals listed on an IACUC research or teaching Animal Use protocol, including all Farm and Equine personnel and students
• Individuals involved in animal care in animal use room housing animal for research and teaching
• Other individuals who may reasonably be expected to come into contact with animals, animal tissues or wastes

All individuals required to participate in the program must complete the Occupational Health and Safety Program Questionnaire.

Important note: The information in the questionnaire is confidential. Completed questionnaires must be personally delivered by each participant to the Research Compliance staff, Velma K. Waters Library, Office of Sponsored Programs, Room 225B.

The questionnaires will be evaluated by the Medical Office at the Student Health Center and all information will be kept private and confidential.
