Exception Request Form
The Exception Requests Form documents the Electronic Information Resources product or service developed or procured that cannot be made fully compliant with the applicable TAC 206 and/or TAC 213 accessibility standards and specifications due to significant difficulty or expense to the University.
The exception request form flows through the department management chain to the EIR Officer, CIO, and president or designees. The form provides a description of the accessibility issue, justification for exception, and plan for alternate means of access. The exception request requires approval by the department head, EIR Officer, CIO and President.
The process of the exception request is as followed:
Step 1: Requestor fills out and submits the online Exception Request Form
Step 2: Department Head reviews the exception request form
- If approved or denied, the form proceeds for review to EIR Officer
Step 3: EIR Officer reviews the exception request form and forwards to CIO
Step 4: CIO reviews the exception request form
- If approved, the form proceeds for review to the President
- If denied, the requestor, department head and EIR Officer will receive an email notification.
Step 5: President reviews the exception request form
- If approved or denied, the department head and EIR Officer will receive an email notification
