Purpose of Form
The purpose of this form is to aid the Accountable Property Officer (APO) in managing equipment and to protect A&M-Commerce against theft or loss of this equipment.
Submission of Form
The APO of each department should complete this form before submitting a copy to the Property Manager. This document will be stored in a Laserfiche repository, accessed by the Property Manager and Financial Services. Departments may choose to keep a PDF copy for their files.
Completing the Form
Complete all three sections of the form. Section one contains information found on the equipment information sheet. Sections II and III need to be completely filled out by the APO also. Additionally, signatures of both the borrower and the APO (department head) are required.
Responsibility of the APO
